      <image:caption>As mentioned prior, sometimes symptoms may be present because of weakness. In this case your physical therapist may focus on the activation and contraction of the pelvic floor during forced exhalation through pursed lips. When you breathe out your diaphragm returns up to its resting state and your pelvic floor lifts or shortens. If tightness or overactivation is present then pelvic floor training may be centered on the inhalation part of respiration, which is when the pelvic floor muscles are lengthening or relaxed. Spoiler alert: kegels are NOT always the answer. Again, at times you can experience both tightness and weakness. While the focus on this article is on the relationship of the pelvic floor with breathing, it is important to note that evaluation by a pelvic floor physical therapist will help determine what technique you need to focus on and at times you may also need manual release, dry needling, manual facilitation or cuing, as well as additional strengthening to optimize your outcomes.</image:caption>

      <image:caption>In Physical Therapy, our craft is centered around goals, movement goals to be exact! So you would have thought I would have easily met my movement goal I set in the new year, of running a 5K in under 30 minutes. I mean, I had done it before although it had been a minute (note pre-kids...8+ years ago). So, in April I set out to run the Boston 5K while my husband was running the Boston Marathon (we all are in different stages of movement). I ran the 5K in 31:16. I didn’t meet my goal…whomp, whomp. It was an awesome experience no matter, running with people from all over the world. With post-race reflection it was no surprise as to why I didn’t meet my goal - I didn’t have a specific plan in place and I didn’t carve out the time for the work that was needed to actually achieve sub 30. I needed some support and accountability (we all need it!)…cue Christine Turner of CT Sculpt. She coached me to build up my base endurance and then she created a plan specific to my goals, honoring where I was in my movement journey and my lifestyle. Having someone tell me exactly what to do and knowing someone was monitoring my progress was a game changer for me. I was able to run the Santa Run 5K in 29:30 and placed 9th in my age group. Whoop, whoop!</image:caption>

Instructions: While barefoot, in comfortable clothing, ‘without worrying about the speed of the movement, try to sit &amp; then to rise from the floor, using the least support that you believe is needed’. Grading separately score the transfer to the floor &amp; rising from the floor on a scale of 0-5 with a highest possible score of 10 starting from 5, one point is subtracted for each support utilized i.e. for each hand, forearm, knee, or side of the leg or foot used. Additional 0.5 point is subtracted if the evaluator notices an unsteady execution (partial loss of balance) during them movement a score of 10 indicates the ability to sit &amp; to rise from the floor without using any support or presenting instability Facts &amp; Figures: a score of 10 is the most frequently seen in men aged 16 to 25 years old &amp; in women aged 16 to 40 years old less than 8% of men and women aged &gt; 55 years old achieved a composite score of 10  Why is it important? Lower performance is strongly associated with all-cause mortality in subjects aged 51 to 80 years. Although this is correlation &amp; not causation, in our expert opinion, your ability to move with ease is definitely associated with a higher quality of life.       <image:caption>Concussions or mild traumatic brain injuries (mTBI) can occur during sports play, car accidents, falls, or with other traumatic events. Direct, blunt trauma to the head does not have to occur, those who have a whiplash like mechanism of injury can also result in concussion. Loss of consciousness is rare but does occur. Sometimes amnesia from an unobserved event like a fall may be mistaken for loss of consciousness. Symptoms are not alway immediate, they can be delayed for hours or even days. For most, symptoms will resolve within 2 weeks but if they persist beyond 2 weeks then discuss the possibility of Postconcussion Syndrome (PCS) with your medical provider so that you may initiate treatment ASAP if you haven’t already done so. This is a silent condition, meaning to your boss, your teachers, your friends &amp; loved ones it may seem like everything is “normal” but if it’s not then it’s important to be an advocate for your health. If you have a provider dismissing your complaints or symptoms then they may not have experience with concussions/mTBI so please listen to your body and look for the correct providers that can help you!</image:caption>

This is a condition when there is an injury to the vestibular nerve, causing dizziness.  In some cases this can be very severe, especially within the first 1-3 days and some will be hospitalized.  Due to the injury, which is usually on one side, there is a change in the input to the brain from the inner ear which results in dizziness. This is another group that do extremely well with therapy, and essentially are “recalibrating” their inner ear and surrounding systems.  Vestibular migraines and concussions are other subgroups commonly seen for vestibular rehab.  Both of these groups present differently and require extensive examinations, looking at multiple systems.  Vestibular rehab includes training of all systems involved – inner ear, eyes, brain, balance, etc. and how they communicate together.</image:title>

      <image:caption>Some are under the impression that PT treatment is only exercise based and while movement is a tool, manual therapy typically initiates the healing process. I was introduced to a number of manual therapy techniques when earning my Doctorate of Physical Therapy, but as Kristin mentions, manual skills are honed with post-graduate practice and continued education. I am Maitland trained which I tend to complement with other techniques. Generally, I believe in correcting any joint or soft tissue restrictions (if present) before implementing exercise. Otherwise you are reinforcing an already present “abnormal” movement pattern that has likely been provoking pain or dysfunction. With neurological diagnoses, I believe manual facilitation is key to train efficient movement patterns from an altered central nervous system. As the restrictions and movement patterns improve, you are encouraged to MOVE and strengthen the muscles that have been identified as weak. This is where your dedication to recommendations and your home exercise program is crucial. This is what makes the intervention “stick” and provide long term management of a condition.
